POSITION SUMMARY
Under the direction of the Director of Personal Health Services, the Public Health Nurse III-Supervisor Winning Start Winnebago promotes and supports population health in Winnebago County by providing leadership, nursing care, and oversight of daily operations of aspects of the Winning Start Winnebago nurse home visitors. The position will supervise a diverse team of public health professionals in the delivery of the core functions and essential services of public health nursing. Public Health Nurses collaborate and consult within multidisciplinary and multi-agency teams in order to address the social determinants of health and achieve desirable outcomes for families and communities. The supervisor assists the staff in achieving their competencies and professional goals, oversees and delivers services directly, which include but are not limited to assessments, clinical assessment, investigations, education, and home visits. In their leadership role, the supervisor actively participates in policy development and quality improvement activities to support the positive development of families.
Public Health Nursing interventions occur at individual, family, community, and system levels depending on the program and issues involved. Specific services may include outreach and engagement of potentially eligible clients, education, assessment, referrals, home visits, and case management to decrease potential negative health outcomes and address the social determinants of health to support optimal outcomes for pregnant women and their infants.
Winning Start Winnebago includes public health nurse home visiting programs delivered under the evidence-based Family Connects International (FCI) model and the High Risk Family Case Management (HRFCM) program.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Education, Licensure, Certifications, Experience
* Registered in Illinois as a Registered Professional Nurse (RN) required.
* Master’s Degree in Nursing or related field required.
* Advanced Practice Registered Nurse - Pediatric Clinical Nurse Specialist (CNS), Pediatric Nurse Practitioner (PNP), Family Nurse Practitioner (FNP), or CNS with Family-focused specialization required.
* Pediatric experience required.
* Two (2) years of experience in program administration/coordination.
* Two (2) years’ experience working with populations served by Public Health programming preferred.
* National certification in Nursing Administration, Public Health, or Community Health preferred.
* Experience in use of multiple healthcare computers system and Microsoft Office Suite.
* Successful completion of WCHD HIPAA and Blood Borne Pathogens Trainings within 90 days of employment.
* Successful completion of the Illinois Department of Public Health’s "OHP Data Security & Confidentially Training" within one 90 days of employment.
* Must participate in assigned trainings, including on-line training in the National Incident Management System (NIMS) offered through the Federal Emergency Management Agency to obtain the following certifications within nine (9) months of employment: ICS 100, ICS 200, ICS 700, and ICS 800
* Current CPR certification.
* Knowledge of best practices in trauma-informed organizations and trauma-informed care.
