*Job Description: Home Health Speech-Language Pathologist (SLP) - PRN to Full-time (Per Visit)*
*Position Title:* Home Health Speech-Language Pathologist (SLP) - PRN (hourly part time possible as desired)
*Reports To:* Director of Homecare
*Location:* Home Care / Patient’s Residence
*Job Type:* PT/FT/PRN Paid Per Visit
*Position Overview:*
The Home Health Speech-Language Pathologist (SLP) will provide high-quality, evidence-based speech therapy services to patients in their homes on a PRN (as-needed) basis. This role involves evaluating and treating patients with speech, language, cognitive, and swallowing disorders, ensuring compliance with the Conditions of Participation (CoPs) as mandated by the Centers for Medicare & Medicaid Services (CMS), and adhering to all state and federal regulations. This position is paid on a per visit basis, with the number of visits varying depending on patient needs and available cases.
*Key Responsibilities:*
* *Patient Assessment and Evaluation:*
* Conduct comprehensive evaluations to assess patients’ speech, language, cognitive, and swallowing abilities.
* Develop individualized care plans based on patient assessments, functional goals, and physician orders.
* Document evaluations, progress notes, treatment plans, and changes in the patient’s condition in compliance with agency policies and regulatory requirements.
* *Treatment Planning and Implementation:*
* Design and implement evidence-based therapeutic interventions to improve patients’ functional communication, swallowing, and cognitive abilities.
* Educate and train patients, caregivers, and family members on speech, language, and swallowing techniques to improve quality of life and facilitate independence.
* Adjust treatment plans based on the patient’s progress and ongoing needs.
* *Compliance with Regulatory Standards:*
* Ensure compliance with the Conditions of Participation (CoPs) outlined in the Home Health Care regulations under CMS, including but not limited to:
* Patient-centered care planning and goal setting.
* Timely and accurate documentation and reporting.
* Adherence to infection control protocols.
* Accurate billing for services rendered.
* Follow federal, state, and local regulations related to home health care and speech therapy.
* *Collaboration with Interdisciplinary Team:*
* Collaborate with other healthcare professionals (nurses, physical therapists, occupational therapists, social workers, and physicians) to coordinate care and optimize patient outcomes.
* Participate in case conferences and interdisciplinary meetings to discuss patient progress and treatment adjustments.
* *Patient and Family Education:*
* Provide counseling and education to patients and families regarding speech and language disorders, treatment options, and strategies for daily living.
* Assist in training caregivers on supporting patients with communication and swallowing difficulties.
* *Quality Improvement and Documentation:*
* Ensure timely and accurate documentation of all patient visits, including assessments, treatments, progress, and any changes in the patient’s condition.
* Participate in quality improvement initiatives to enhance patient care and outcomes.
*Requirements:*
* Valid Ohio licensure as a Speech-Language Pathologist..
*Compensation:*
* This is a PT/FT/PRN (per visit) position. Compensation is paid on a per-visit basis, and rates are based on the type and complexity of services provided.
*Working Conditions:*
* Position requires travel to patient homes, with potential exposure to various environmental conditions.
* Flexible hours based on patient needs and availability, with evening and weekend visits possible.
