Occupational Therapist - OT
Location: Leo-Cedarville & Fort Wayne, Indiana
Schedule: Full-time (30-40 hours per week, Monday through Friday)

Healthcare Therapy Services is seeking an Occupational Therapist to join our multidisciplinary teams at two well-regarded facilities located in Allen County, Indiana (Fort Wayne and Leo-Cedarville). This OT will work with diverse populations, addressing their unique therapeutic needs. At 30 – 40 hours per week, this is a FULL-TIME position. Drive time and mileage is compensated for travel between the two facilities, located 15 miles apart. May also work a schedule of 3 days/week at the primary facility and 2 days/week at the secondary.
Primary Facility
· Tight-knit community located in Leo-Cedarville.
· Continuing Care Retirement Community with skilled nursing, assisted living and independent living. Memory care coming soon.
· Well-equipped therapy gym has a training kitchen and bathroom.
Secondary Facility
· Wellness community located near Parkview Hospital in Fort Wayne.
· Special care services for young and older adults from skilled nursing, assisted living and memory care.
· Large therapy area has a variety of equipment and a training bathroom.
Healthcare Therapy Services is also seeking an Occupational Therapist to join our multidisciplinary team at a well-regarded senior living community located close to Parkview Randallia Hospital in Fort Wayne, Indiana. This OT will work with diverse populations – including assisted/independent living, memory care, short term rehab, and long-term care - addressing their unique therapeutic needs.
·Well-equipped therapy gym with training kitchen and bathroom.
·Collaborative team serving robust rehab-to-home and outpatient populations.
·Referrals supported by close ties to a nearby major hospital and the community.

Requirements:
Applicants must have an active Indiana Occupational Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include a degree from an ACOTE-accredited occupational therapy program, subsequent passing of the National Board of Certification in Occupational Therapy (NBCOT®) examination and fulfilling the state’s requirements for licensure.
