About
The Grande Prairie Primary Care Network is seeking a Licensed Practical Nurse (LPN) to be a key member of the PCN's After-Hours Clinic (AHC) team. The LPN performs work as required to support patient care, providing clinical support to Physicians and Nurse Practitioners.
This is a permanent position that works evenings and weekends.

Duties & Responsibilities
- Work collaboratively within a clinical environment to enhance the delivery of primary care services
- According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
- Assist in the implementation of strategies for improving the quality and provision of care for the practice population
- Obtain patient health history
- Complete comprehensive assessments and document observations
- Review and order lab work as directed by physician or nurse practitioner
- Review medication and assist with prescriptions as directed by physician or nurse practitioner
- Administer medications, treatments, and procedures.
- Organize consultations and further investigations.
- Assist with follow up care.
- Document patient assessment, treatment information, reports, referrals, etc. and maintain patient charts.
- Collaborate and communicate effectively with AHC physicians, nurse practitioners, team members and internal/external resources.
- Assist in development of clinical policies and protocols.
- Ensure the maintenance of accurate records in line with practice policy.
- Maintain current knowledge and skills in clinical practice.
- Participate in quality improvement activities.
- Perform clinical assessments as directed by physicians (i.e. BP, weight, height, temperature pulse, respirations, glucose levels, etc.)
- Schedule patients for follow-up tests and specialist appointments if requested by physician.
- Sterilize instruments and ensure infection control guidelines are followed.
Required Skills:
- Strong clinical skills in Primary Care nursing
- Strong computer skills required, including knowledge of and ability to use electronic medical records.
- Ability to function cooperatively with physicians and clinic staff.
- Ability to build a good working relationship with patients, families and other healthcare professionals.
- Ability to develop new skills and apply theory to practice.
- Respect and understanding of confidentiality.
- Excellent verbal and written communication skills
- Ability to work on own initiative and to prioritize workload.
Required Qualifications:
- Current Alberta Association of Registered Nurses or College of Licensed Practical Nurses of Alberta Registration
- Current CPR certification
- Degree or diploma in nursing
- Minimum of 1 year's nursing experience
- Experience in primary care/community nursing and walk-in clinics preferred
