Responsibilities- Following a pre-set care plan
- Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
- Assist with medication reminders
- Monitor and report on client's well-being
- Light housekeeping, laundry, and other household daily chores as needed
- Grocery shopping and errands
- Cooking/meal preparation
- Scheduling appointments, including doctor's
- Communication with Family Ties HC management and family members/loved-ones regarding the client's daily status
- Daily notes
- Other duties assigned as needed
- Current HCA or higher certification
- Caregiving experience of 1 year or similar experience
- Compassionate and caring demeanor
- Excellent communication & interpersonal skills
- Ability to follow the pre-set care plan and medication schedule
- Ability to lift 35 pounds
- Ability to assist with client's mobility as needed
- Valid driver's license and reliable transportation
- Ability to work the schedule as described
