Occupational Therapist

CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home therapy services. In this role, you will evaluate patients’ functional abilities, create individualized treatment plans, and provide evidence-based interventions to improve safety, independence, and quality of life. You’ll conduct home safety assessments, recommend adaptive equipment, document care in EMR, and educate patients and caregivers. Working in a collaborative, patient-centered team, you’ll support excellent clinical outcomes while upholding our values of compassion, integrity, and continuous improvement.
