Hospice Practice Overview:

* Dedicated hospice team serving patients for 40 years.

*Inpatient Hospice Unit at main campus- 7 bed unit

* Free standing 16 bed hospice currently under construction just minutes from main campus

* Physicians support nursing and interdisciplinary staff by providing oversight for hospice and in-home palliative care

* Desirable schedule Monday through Friday schedule

*Service Line supported by dedicated:

o Advanced Practitioners

o Licensed Clinical Social Workers

o Chaplains

o Designated nurse staffing

* Growing program with opportunity to work with residents and fellows

Working together with Physicians like you:

* Board-Certified or Board-Eligible in Hospice & Palliative Medicine (HPM)

*Active Georgia license or ability to obtain unrestricted license

*Experience providing care across settings including inpatient consults, hospice inpatient units, home hospice, and outpatient palliative care

* Passion for quality and patient care

* Strong communication skills and comfort with goals-of-care discussions, symptom management, and end-of-life care planning

*Functions well in a team environment, contributing to and supporting organizational goals and initiatives

* Physicians holding legacy or time-limited HPM certification remain fully acceptable, provided certification is maintained per governing board requirements
