About
Monitor and interpret patient vitals, assessments, and alerts within Welby Health’s platform Conduct virtual patient assessments, medication reviews, and care plan evaluations Provide timely, evidence-based guidance and escalate clinically significant findings to supervising physicians when appropriate Document patient interactions accurately and completely in accordance with internal protocols and regulatory standards Coordinate with care teams and physicians to ensure seamless care delivery and continuity Support quality improvement initiatives through consistent, high-quality clinical documentation and adherence to established workflows Required Qualifications
Active and valid nurse practitioner (NP) license in the state of Florida Board certification as a family nurse practitioner (FNP), adult-gerontology nurse practitioner (AGNP), or equivalent Eligibility for Medicare provider enrollment and participation Bachelor’s degree in nursing or a closely related healthcare field Exceptional written and verbal communication skills Ability to work independently in a remote, technology-enabled environment Strong organizational skills with attention to detail and follow-through Preferred Qualifications
Experience in case management, chronic care management, and/or telehealth Multi-state licensure for expanded practice flexibility Drug Enforcement Administration (DEA) licensure Bilingual or multilingual proficiency Familiarity with remote physiological monitoring, electronic medical record (EMR) or electronic health record (EHR) systems, and patient engagement tools Compensation
This independent contractor role offers flexible, volume-aligned compensation structures that may include per-visit rates, hourly rates for scheduled clinical coverage, or hybrid models as patient volume scales. Compensation is commensurate with visit complexity, clinical coverage requirements, and performance. About Us
