Centralized Plan Coordinator Full-Time Position Join our team and make a meaningful impact in the lives of individuals with developmental disabilities. As a Centralized Plan Coordinator, you will partner with people receiving services and their support teams to develop, implement, and update individualized goals and service plans. This role plays a key part in ensuring person-centered services are delivered consistently and effectively. Position Summary The Centralized Plan Coordinator oversees the planning, coordination, and documentation of services based on person-centered planning principles. This position ensures that all aspects of each individual's program plan are implemented, monitored, and documented in accordance with agency standards and regulatory requirements. Qualifications
Bachelor's degree in Human Services or a related field Minimum of one year of experience supporting individuals with developmental disabilities Ability to meet Qualified Intellectual Disabilities Professional (QIDP) eligibility requirements Valid New York State driver's license with a driving record acceptable to the agency's insurance carrier
Preferred Skills
Strong organizational and documentation skills Excellent communication and interpersonal abilities Ability to collaborate effectively with individuals, families, and interdisciplinary teams Commitment to person-centered practices and advocacy
