The Ambulatory Operations Pharmacist partners with ambulatory clinics to identify, investigate, and resolve medication errors and near misses; assist with medication safety initiatives; and develop, implement, and maintain medication-use policies and procedures. The role also supports optimal use of the EMR/EHR (e.g., prescribing workflows, clinical decision support, order sets) and coordinates ambulatory-related projects to improve safety, quality, and operational performance across outpatient/ambulatory settings. Key responsibilities include:
Receive, triage, and document medication errors/near misses originating in ambulatory clinics; perform root-cause analysis and coordinate corrective and preventive actions. Collaborate with providers, nursing, clinic leadership, and outpatient pharmacy teams to design and implement medication safety improvements (e.g., prescribing, dispensing, administration, monitoring, and patient education workflows). Draft, review, and maintain ambulatory medication-use policies, procedures, guidelines, and order set standards; ensure alignment with regulatory requirements and organizational governance. Lead or support medication safety committees, safety event reviews, and quality improvement projects; track trends, metrics, and outcomes and communicate findings to stakeholders. Support ambulatory pharmacy and clinical pharmacy operations, including workflow optimization, standard work development, staff education, and implementation of new services or practice changes. Provide subject-matter expertise on high-alert medications, medication reconciliation, transitions of care, and safe use of health information technology (e.g., EHR prescribing and clinical decision support).
Qualifications include:
PharmD and active pharmacist license (as required by the jurisdiction). Experience in ambulatory care, medication safety, quality improvement, and/or policy development; residency training (PGY1/PGY2 Ambulatory Care) preferred. Strong collaboration, investigation, writing, and change-management skills; ability to work across multidisciplinary clinic teams. May require weekend rotations.
