Deliver primary care services to adult patients, including health screenings, diagnostic evaluations, and therapeutic interventions Perform detailed medical assessments and formulate individualized care plans Facilitate coordination of services and specialty referrals in partnership with clinical staff Complete thorough and timely medical records documentation and engage in periodic chart audits Work alongside clinical leadership to enhance care delivery processes and overall service excellence Job Requirements: Job Requirements:
New York State license, DEA, CSR preferred
Masters Degree as a Nurse Practitioner
2 plus year experience as Nurse Practitioner
