We are seeking an experienced and detail-oriented MDS Coordinator to join our skilled nursing team in Delaware, Ohio. This is an excellent opportunity for a nurse who is passionate about resident-centered care, clinical excellence, and maximizing reimbursement accuracy through effective MDS management. As the MDS Coordinator, you will play a key role in coordinating the Resident Assessment Instrument (RAI) process, ensuring regulatory compliance, and collaborating with an interdisciplinary team to deliver exceptional outcomes for residents. What You'll Do
Coordinate and complete MDS assessments in accordance with federal and state regulations Manage the RAI process and ensure timely completion of assessments and care plans Monitor Medicare, Managed Care, and Medicaid reimbursement opportunities Collaborate with nursing, therapy, dietary, social services, and other departments to ensure accurate documentation Conduct chart reviews and audits to support compliance and reimbursement accuracy Participate in care plan meetings and interdisciplinary team discussions Stay current with regulatory changes and reimbursement guidelines Support quality measures and clinical outcomes initiatives
Qualifications
Current RN license in the State of Ohio Previous MDS Coordinator experience preferred Strong understanding of RAI/MDS processes and Medicare reimbursement systems Knowledge of PDPM and skilled nursing facility regulations Excellent organizational, analytical, and communication skills Ability to work independently while collaborating effectively with interdisciplinary teams
