Position Objective

To provide services to individuals receiving Supported Living services to help individuals with all abilities in reaching their goals and gaining independence.

Job Overview

The Direct Support Professional will be responsible for providing excellent, quality care to the individual he/she serves. The Direct Support Professional will be required to teach the individual daily living skills and assist them in working toward their individual goals/objectives made by their team. The daily tasks that a Direct Support Professional may be required to do includes assisting in personal care, goal-setting, transportation of individuals to appointments/outings, assisting during dining when needed, among other tasks. The Direct Support Professional will also be responsible for ensuring all required documentation is completed within the allotted time frame and communicate needs/concerns regarding the individual to their supervisor.

Primary Job Duties and Responsibilities
  • Provide for the individual's daily care that may include, but is not limited to:
    • Assist with bathing, dressing, and grooming
    • Assist with dining needs
    • Assist with transferring and ambulation
    • Teach/model/encourage communication/social skills
  • Encourage individual to complete goals/objectives daily
  • Offer emotional support to the individual and work with the family
  • Communicate and actively listen
  • Understand and implement individual's Person Centered Individual Support Plan (PC-ISP) and help individual reach their objectives/goals daily per plan
  • Must follow Universal Precautions techniques in accordance with all agency policies and procedures to include use of personal protective equipment
  • Maintains an environment that is healthy, safe, peaceful, and comfortable
  • Gains and maintains knowledge of health and medical issues for all assigned individual(s) supported
  • Assists individual with maintaining routine health care visits, medications, and therapy plans, and transports individual as needed
  • Notifies appropriate supervisor of any health concerns or needs for the individual supported
  • Follows all policies and procedures regarding safety in all environments
  • Transports individual to appointments/outings/office as needed/scheduled
  • Completes all necessary/required paperwork in the allotted time frame
  • Other direct care tasks as assigned
Qualifications and Education
  • High school diploma/GED required
  • Valid driver's license and reliable vehicle
  • No previous criminal record
  • Current First Aid and CPR certification (Offered by Sky Point, if needed)
  • Excellent organizational, interpersonal, and communication skills
  • Compassionate about the care and well-being of others
  • Ability to pass a background check and drug test
  • Smartphone with data with GPS capabilities
Location
  • Job site is located in and around Northeast Indiana
  • Applicants must be able to travel to the Fort Wayne office for the new-hire process, orientation, and, as needed, to stay compliant.
