We are currently looking for FT/1st Outpatient Physical Therapist. This position is responsible for the evaluation, plan of care development, and provision of skilled physical therapy services across acute care and outpatient setting in accordance with hospital policy and applicable stat practice acts. The physical therapist ensures that all services are medically necessary, goal-oriented, and functionally based, with documentation that supports skilled intervention and regulatory compliance. The role supports safe patient progression across the continuum of care and contributes to interdisciplinary planning and quality outcomes.
*ESSENTIAL FUNCTIONS: *
1. Performs initial evaluation within 24 hours of referral excluding holidays and weekends
2. Provide safe and effective interventions
3. Progresses patients toward functional goals
4. Documentation supports medical necessity and skilled care
5. Develops appropriate and measurable goals; updates plan of care appropriately.
6. Establish effective relationships with physicians, nursing staff, and leadership.
7. Communicates effectively with team members and care teams.
8. Actively participates in discharge planning and supports coordinated patient care.
9. Ensure compliance with ACHC and other regulatory requirements.
10. Adheres to infection control and safety standards
11. Implements fall prevention and safe patient handling.
12. Contributes to quality improvement initiatives.
13. Maintains licensure and competencies.
14. Oversees daily department operations effectively
15. Ensure staff licensure and credentialing compliance.
16. Effectively manages operations, staffing, and workflow.
17. Works effectively with interdisciplinary teams.
18. Ensures compliance with safety standards.
19. Maintains licensure, ethics, and leadership presence.
*Required:*
* Graduate of Bachelors, Masters, or Doctor of Physical Therapy from an accredited program
* Bachelor’s Degree in Physical Therapy
* Licensed Physical Therapist in State of Illinois
* BLS from American Heart Association upon hire
