About
• Reports changes in patient's condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor. • Applies AIDET as well as other evidence-based behaviors. • Participates in developmental and organizational initiatives for process improvement and quality assurance as requested. • Follows appropriate infection control procedures. • Performs and accurately documents the following procedures as directed by the RN / LPN: a. Blood glucose checks and documentation b. Discontinuation of urinary catheters c. Discontinuation of peripheral IVs d. Performs skin care interventions and treatments, as permitted by state regulation. • Maintains patient and staff confidentiality. • Assists in the orientation of new staff as requested. • Performs other duties as assigned. Unit Specific Responsibilities:Med/Surg, Peds, PCU, ICU, Observation Unit. and ER: • Maintains educational requirements and training requirements to fulfill the role and responsibilities of a Unit Coordinator as outlined in the Unit Coordinator job description. • Regularly updates patient communication boards. • Performs hourly rounds with patients. • Performs bedside shift report. • Assures patient belongings are accounted for on admission and transfers and returned at discharge. • Performs designated admitting tasks. • Records accurate intake and output for each patient. • LNA may be required to provide backup support a Camera Tech in specific units: a. Demonstrates competence with operating video monitor equipment by managing the video monitor (including audio component), display of rooms, adding and deleting beds monitored; is aware of actions for video monitor Downtime. b. Receives report on video patient assignment from nurse. Stays at the monitor at all times, reports changes in patient's behavior/status to nurse. c. Documents and signs "Record and Video Monitoring form" (verbal reminders, staff interventions, # of fall alerts. d. Identifies the behaviors that may precipitate a fall; is aware of the proper steps to take if patient is not seen on the video monitor. Home Health and Hospice: • All documentation is completed accurately and timely (no later than 8am the next day). • Productivity to average no less than 5-6 patients per day. Hospice House: • Performs hourly rounds with patients. • Performs bedside shift report. • Assures patient belongings are accounted for on admission and transfers and returned at discharge. • Performs designated admitting tasks. • Records accurate intake and output for each patient. • Performs designated admitting tasks. • Must be 18 years of age or greater Experience: Previous experience as a Nursing Assistant, OR successful completion of a Nursing Assistant certification program Education: High School or equivalent Licensure: Current valid Arizona LNA License Certification: Current BLS through the American Heart Association BLS or American Red Cross Unit Specific:Pediatrics: • Two-week pediatric orientation with annual competency in pediatrics Home Health: • Level one fingerprint clearance card, or application, required within 20 days of hire. • Current and valid Arizona Driver's License and clean driving record. Hospice and Hospice House: • Level one fingerprint clearance card, or application, required within 20 days of hire. • Current and valid Arizona Driver's License and clean driving record. • Current and valid Arizona Food Handlers Card.
