Nurse Practitioner
(Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join a collaborative team serving residents in long-term care and skilled nursing settings.

What You ll Do:
Evaluate, diagnose, and manage the ongoing health needs of residents in a long-term care or skilled nursing facility Collaborate with an interdisciplinary care team to develop and adjust individualized treatment plans Provide compassionate, person-centered care that improves quality of life for seniors Maintain clear, timely documentation and communicate effectively with facility staff and family members What We re Looking For:
Active Nurse Practitioner license (APRN) FNP or AGNP Experience in geriatrics, long-term care, or primary care preferred (new graduates encouraged to apply) Commitment to patient-centered care and strong communication skills Ability to work independently and as part of a supportive care team What s Offered:
