About
Provide direct resident care including bathing, grooming, dressing, toileting, and oral hygiene. Assist residents with mobility, transfers, repositioning, and ambulation. Answer call lights promptly and ensure residents' needs are met in a timely manner. Measure and record vital signs (TPRs), weights, intake and output as directed. Observe and report changes in residents' physical or emotional condition to the Charge Nurse. Assist residents during meals, including documenting food and fluid intake. Maintain clean and organized resident rooms and ensure residents remain dry and comfortable. Assist with admissions, transfers, and discharges. Perform restorative and rehabilitative procedures as instructed. Follow all infection control and safety protocols, including bloodborne pathogen procedures. Accurately complete documentation and charting in accordance with facility policies. Maintain strict confidentiality of all resident information in compliance with HIPAA. Immediately report incidents, injuries, abuse allegations, or safety concerns. Follow all established policies and procedures and perform duties as assigned by supervisors. Supervisory Responsibilities This position does not have supervisory responsibilities. Qualifications Education and Experience
Minimum 10th grade education required. Previous experience in a skilled nursing facility preferred. Certificates, Licenses, Registrations
Current, active Certified Nursing Assistant (CNA) license in the State of California. CPR certification preferred. Required Skills and Abilities
Ability to communicate effectively with residents, families, and staff. Ability to follow written and verbal instructions. Basic math skills for documentation and intake/output recording. Ability to recognize and report changes in resident condition. Professional demeanor and commitment to compassionate care. Physical Requirements
Frequent standing, walking, bending, lifting, and repositioning residents. Ability to lift and/or move up to 25 pounds, and assist with heavier transfers as part of a team. Ability to respond quickly in emergency situations. May assist in resident evacuation during emergencies. Work Environment This position works within a skilled nursing facility environment with exposure to healthcare-related conditions. The noise level is typically low to moderate.
