MPNN BH has over 60 distinct programs to aid and support all ages and many situations. Job Summary The High Fidelity Wraparound process is a needs based planning process to improve the lives of children with complex mental health needs and their families by empowering the families and youth to have their voices heard through an individualized planning process. The individualized plan is child-centered and family-focused with the process focusing on strengthening the natural family, extended family and social supports for the child and family by developing a team to support the family throughout the process and in learning the skills of the model. The Parent Support Partner works with the parents (and families during meetings) who have agreed to participate in the Intensive Care Coordination Process. The Parent Support Partner will work as part of the Child & Family Team providing support and guidance to the parents throughout the process, during meetings, and in completion of the individualized plans developed by the youth and their family. The Parent Support Partner will work with the parents of the identified youth to develop their own team of supports (natural, community, and systems). The Parent Support Partner will use their own life experiences to model for the parents how to navigate systems using their voice to meet the needs of their child while developing natural supports within their own community to reach their family vision. Job ResponsibilitiesServe families with children through age 21 that have a serious emotional disturbance that is diagnosable under the DSM-IV. Population to be served will have one or more of the following: a mental health problem, a co-occurring mental health and substance abuse problem, contact with the social services system, juvenile justice or court system, require emergency services, or require long term community mental health and other supports. Use life experience to support families in navigating VA behavioral health systems and other formal and informal community resources Ensure parents/caregivers understand the process of High Fidelity Wraparound and understand the roles of team members and the information presented to them Work collaboratively with Intensive Care Coordinator to provide High Fidelity Wraparound care for identified clients. Become an active member of the Youth and Family Team in order to support youth and families as they identify and engage with formal services and informal supports. Assist the youth, family, wraparound facilitator and team in the strengths and needs discovery, and promote and reinforce strength-based strategies. Successfully complete training in High Fidelity Wraparound Family Support Partner. Build trust with parents/caregivers by engaging in a confidential, respectful, non-judgmental, and supportive relationship. Listen to and validate each family member's perspective, feelings and values allowing everyone to be heard. Support the implementation of the 10 principles of High Fidelity Wraparound and the implementation of the 4 phases of High Fidelity Wraparound. Ensure that the principles and phases are delivered with model fidelity. Help families understand their child's needs and services. Role model how parents/caregivers should address controversial information for a successful outcome. Follow the Plan of Care developed in team meetings to help families achieve their family vision. Assist family with completing their assigned action steps that are assigned in the Plan of Care. Participate in providing a variety of support services for parents/caregivers such as support groups, family activities, etc. as assigned in the Plan of Care. Model how to advocate for self and empower them to use voice and choice in planning decisions for their child. Transfer skills to help families learn how to engage in self-care practices so that they can sustain positive parenting roles. Attend all Youth and Family Team meetings, typically held monthly but may be more often as necessary, and assist the youth, family and wraparound facilitator in meeting preparation. Attend other meetings as needed and as requested by the youth and family (i.e. FAPT, IEP, ect). Support the voice and choice of the youth and family through modeling the effective presentation of ideas and suggestions during team meetings. Assist family and youth in identifying and developing natural supports. Act as a liaison between families and professionals. Maintain a working relationship with professionals. Have timely and accurate documentation. Other responsibilities as assigned. RequirementsJob RequirementsEssential • Must pass all required Background Checks. • Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
• Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority. Education and Experience Parent or caregiver of a child who has received behavioral health services from one or more of the System of Care community's child serving agencies. Knowledge and experience in fostering partnerships and collaborations. Demonstrated experience collaborating with diverse stakeholders. Some understanding of the System of Care Values and principles. Must have the ability to articulate the experience and appropriate perspective of a parent/caregiver of a child with complex mental health needs and provide support to other families with similar challenges. Must be able to engage and collaborate with people from diverse backgrounds while maintaining a non-judgmental attitude towards families and professionals. Bi-lingual (Spanish) speaking preferred. High school diploma and some college preferred. Must be the biological or adoptive parent or person in the parent role who has been the primary caregiver of a child with emotional, behavioral or mental health challenges is required.
