About
Training: Initial & Ongoing training in Anticoagulation Management including interpersonal communication. Opportunities to complete USI certificate course and, in future, anticoagulation certification. Education of future health care providers: Precept students and PGY1 pharmacy residents. Expose anticoagulation management to PGY1 internal medicine medical residents. Quality Improvement: Update and develop system-wide anticoagulation protocols and initiatives to enhance patient care.
This full-time position currently offers a balanced work-life schedule:
Monday - Friday, between 7:30 am - 5 pm. Saturdays in a multi-person rotation. No evenings, Sundays, or holidays.

Bachelors of Science degree in Pharmacy or Doctorate Degree in Pharmacy required. ASHP-Accredited PGY-1 Pharmacy residency or 3 years of relevant clinical pharmacy experience required. Active and valid PA licensure or eligibility as a Pharmacist required.
