About
1. Identify correct provider orders in Epic; accurately interface them to the laboratory information system (LIS), Orchard. * 2. Complete order details in the LIS including clinical information, copy to provider, fasting, comments, etc. Generate Orchard labels to identify patient and collect/label samples per protocol. * 3. Collect blood from patients (infant to elderly) using syringes, Vacutainers and butterflies. * 4. Provide clear verbal or written instruction to patients for the following collections: urine, sputum, stool. Capably collect throat swabs. * 5. Prepare all samples and route them to the correct laboratory for testing: the branch, Downtown core laboratory or reference laboratory. * 6. Centrifuge and separate blood as necessary; store at the correct temperature 7. Package specimens according to protocol and destination 8. Perform and record quality control testing for the following procedures: rapid screens for Strep, mono, influenza and UCG; fingerstick glucoses and hemoglobin; urinalysis (dipstick and microscopic), HSV, VZV, etc.* 9. Follow procedures accurately while completing all analyses including: sed rates, all rapid tests (influenza, UCG, mono, influenza), dipstick and microscopic urinalysis, VZV, HSV (based on IQCP), 10. fingerstick hemoglobin and glucoses. * 11. Perform proficiency testing for appropriate assays. 12. Maintain lab records (i.e. quality assurance and quality control) by completing documentation. * 13. Ensure adequate inventory of lab related supplies by using Materials Management or contacting the Downtown laboratory or appropriate reference laboratory. * 14. Demonstrate a cooperative, approachable manner with management, coworkers, and customers. * 15. Consistently display courtesy and respect to any customer or member of the public while in the course and scope of company business. * 16. Participate as an active, helpful, team member in a patient-centered medical home. * 17. Answer phones; communicate professionally, provide assistance and/or transfer calls when needed. * 18. Work effectively at any branch scheduled (South, Beaverton, Northeast, Tigard, Downtown). * 19. Clean phlebotomy equipment and lab surface every day and as needed. * 20. Assist with disposal of biohazard waste following appropriate safety protocols. * 21. Maintain current CPR certification. * 22. Present a positive attitude while following all company policies. * 23. Other duties as assigned. * 24. Maintain regular, in person, work attendance and punctuality, as scheduled.* REQUIREMENTS:
• High school diploma or equivalent • Current CPR certification • One year of experience in phlebotomy, OR completion of an accredited phlebotomy training program, OR one of the following certifications: ASCP (PBT), NHA (CPT), AMT (RPT), NCCT (CPT) or equivalent.
