Your Role and Responsibilities
Assess patients in a variety of settings, crafting personalized and effective treatment plans Implement therapeutic interventions that enhance mobility, strength, and overall function Work collaboratively with an interdisciplinary team comprising nurses, physicians, case managers, and rehabilitation professionals to achieve the best patient outcomes Instruct patients and their families in understanding therapeutic objectives, developing discharge plans, and conducting home exercises Maintain comprehensive documentation regarding evaluations, treatment protocols, and patient progress in alignment with established guidelines Engage in team discussions and case conferences to facilitate cohesive and coordinated patient care Qualifications and Experience
A Doctorate in Physical Therapy (DPT) from an accredited institution is required Licensure and Certifications
Must hold a valid state licensure as a Physical Therapist CPR certification preferred, additional specialized certifications might be advantageous (e.g., neurological, geriatric) Key Skills
Prior experience preferred, especially in treating patients with intricate medical or mobility issues Excellent communication and teamwork abilities are essential Proficiency with digital documentation and clinical systems is expected Benefits Provided
