Provide initial and continued assessment of nursing needs of the Veteran. Teach the Veteran and/or Caregiver effective and efficient ways to manage health and care in the home. Monitor the Veteran's condition and the care provided in the home. Oversee the provision of direct care. May delegate tasks to the HBPC LPN depending on the patient's medical needs. Visit the Veteran monthly for the first 90 days the Veteran is enrolled in the HBPC program, then will visit at least every 60 days if supplemented by LPN visits so that nursing visits are provided on a minimum of once a month; will visit monthly if no supplemental LPN visits. Complete the appropriate clinical reminders/screening. Monitor the Veteran's physical and psychosocial status. Notify the Primary Care Provider of changes in condition or care needs. All other duties as assigned.
