Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating tailored treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Engage collaboratively with nursing staff, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes Provide education to patients and their families regarding therapeutic objectives, discharge plans, and essential home exercises Accurately document evaluations, treatments, and patient progression per regulatory and facility guidelines Contribute actively in care team discussions and interdisciplinary conferences to guarantee a seamless continuum of care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
Current state licensure as a Physical Therapist CPR certification is preferred Additional specialized certifications (neurological, geriatric) are advantageous but not mandatory Skills
Demonstrable experience is strongly preferred Able to address patients with intricate medical and mobility challenges Excellent communication and collaboration abilities Proficient with digital charting and clinical documentation systems Benefits
