Monday-Friday 9:00 AM - 5:30 PM Rotating On-Call Schedule (Approximately Every 4-5 Weeks) Job Type Full-Time | Permanent | Direct Hire Location Calvert County, Maryland Position Overview A respected nonprofit healthcare organization is seeking a compassionate Palliative Care Nurse Practitioner to provide high-quality supportive care to patients living with advanced and complex illnesses. This role offers the opportunity to build meaningful relationships with patients and families while helping improve quality of life through symptom management, education, and advanced care planning. The Nurse Practitioner will primarily conduct home visits, typically seeing up to four patients per day, while also providing care in assisted living facilities, nursing facilities, rehabilitation centers, outpatient clinics, and other residential settings. Providers have flexibility in managing their schedules and geographic coverage areas. Responsibilities
Conduct initial consultations and follow-up visits Assess symptom burden and develop individualized treatment plans Lead advanced care planning discussions and family meetings Provide patient and caregiver education Manage medication refills and symptom control strategies Collaborate with physicians, nurses, social workers, and other interdisciplinary team members Respond to patient, family, and staff clinical questions Maintain accurate documentation and billing Participate in on-call coverage as assigned Requirements
Active Nurse Practitioner license Family, Adult, Geriatric, Oncology, or related NP certification Minimum of 2 years of Nurse Practitioner experience required Must have experience caring for seriously ill adult patients Hospice or Palliative Care experience preferred DEA and Controlled Substance Registration in good standing Strong communication, relationship-building, and clinical assessment skills Ability to work independently and manage a flexible schedule Nurse Practitioners only (Physician Assistants not being considered) Why Join This Team?
