About
Providing holistic, patient-centered palliative care to individuals with serious illness and their loved ones in the greater Grand Rapids, MI area. Conduct thorough assessments, develop and manage individualized care plans, and perform ongoing follow-up visits. Lead and support symptom management (e.g., pain, dyspnea, nausea, anxiety) using evidence-based practices. Facilitate goals-of-care conversations, advance care planning, and support informed decision-making aligned with patient values and preferences. Communicate effectively with patients, families, caregivers, and referral sources to ensure continuity and quality of care. Provide education to patients, families, and staff regarding disease progression, treatment options, and supportive care strategies. Prescribe medications and therapies in accordance with scope of practice and collaborating physician agreements. Document all patient encounters accurately and timely in the electronic medical record to support care coordination, compliance, and quality outcomes. Participate in case conferences, team meetings, and quality improvement initiatives and support care transitions across settings, including coordination with hospice team
Qualifications:
Current Nurse Practitioner license in the state of Michigan required. Possesses and maintains DEA Certification for prescribing controlled substances in accordance with delegation of authority from the collaborating physician. Skilled in symptom management and complex care coordination and working both independently. Flexibility in adapting to the changing needs of patients and families. 2 years of palliative care, hospice, or home care experience required. Benefits:
