Conduct patient evaluations in both inpatient and outpatient environments, crafting bespoke treatment plans based on individual needs Implement physical therapy interventions to focus on mobility, strength, balance, and functional abilities Engage in collaborative practice with nursing staff, physicians, case managers, and fellow rehabilitation experts to enhance patient outcomes Educate patients and their families about therapeutic objectives, discharge procedures, and home exercise programs Record assessments, treatments, and progress in line with facility protocols and regulatory requirements Take part in interdisciplinary conferences and care team rounds to facilitate coordinated and comprehensive care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution Certifications
State licensure as a Physical Therapist CPR certification preferred Specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory Skills
Preferred prior experience in the field Capacity to provide treatment for patients with intricate medical and mobility needs Excellent communication and teamwork capabilities Familiarity with digital charting and clinical documentation tools Benefits
