Position Summary The Licensed Practical Nurse (LPN) provides direct patient care and nursing support services within long-term care and skilled nursing environments. The ideal candidate is committed to delivering compassionate, resident-focused care while supporting patients with medical, behavioral health, and mental health needs. This role promotes a safe, respectful, and recovery-oriented environment for residents and their families. Key Responsibilities
Provide nursing care within the scope of practice established by Pennsylvania LPN licensure requirements. Monitor, assess, and document resident conditions, observations, and responses to treatment. Administer medications, treatments, and nursing interventions in accordance with facility policies and applicable regulations. Collaborate with nurses, providers, therapists, and other interdisciplinary team members to ensure quality patient care. Maintain accurate and timely medical documentation. Protect resident privacy and confidentiality in accordance with HIPAA and facility policies. Build positive, professional relationships with residents, families, visitors, and colleagues. Participate in orientation, continuing education, and training programs as required. Follow all facility, state, and federal regulations, policies, and procedures. Promote a safe, respectful, and inclusive care environment.
Qualifications
Graduate of an accredited Practical Nursing program. Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Pennsylvania. Minimum of one (1) year of LPN experience. Current BLS certification (American Heart Association compliant). Successful completion of all pre-employment screening and clearance requirements, including background check and fingerprinting. Strong communication and interpersonal skills. Ability to work effectively as part of a healthcare team. Basic proficiency with Microsoft Office applications. Ability to accurately collect, document, and communicate patient information. Understanding of behavioral health and age-related care.
