Assess and evaluate patients, creating personalized treatment plans tailored to their needs in both inpatient and outpatient settings. Deliver physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall functionality. Work collaboratively with a team of nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes. Educate patients and their families about therapeutic objectives, discharge planning, and at-home exercise regimens. Document evaluations, treatments, and patient progress in adherence to facility and regulatory guidelines. Engage in interdisciplinary team meetings and care rounds to ensure seamless patient care coordination.
Your journey toward becoming a Physical Therapist requires at least a Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution. Alongside that, state licensure as a Physical Therapist is mandatory, while CPR certification is preferred. Any specialized certifications (for instance, in areas such as neurology or geriatrics) would be considered additional assets, though not essential. Ideal candidates will have prior experience, showing their capability to manage patients with varying complexities in medical and mobility challenges. Exceptional communication and teamwork abilities are essential, and comfort with digital charting and clinical documentation systems will serve you well in this role.
