Position: RN Case Manager / Registered Nurse, Case Management (Travel Nursing Job)
Weekly Estimated Pay: $2,470 - $2,744
Location: Silverdale, WA 98383 (Puget Sound, Pacific Northwest)
Setting: On-site at a leading medical center - acute care / hospital
Contract Duration: 13 weeks
Start Date: June 8, 2026
Schedule: Monday - Friday (Day shift)
Shifts Per Week: 5 • Shift Length: 8-hour days • Hours/Week: 40
Electronic Medical Record: Cerner (Cerner EMR)
Required Qualifications - RN Case Manager / Case Management RN Active Registered Nurse (RN) license in Washington or ability to obtain prior to start
Minimum 1 year clinical experience in Case Management, care coordination, discharge planning, or a related RN specialty
Experience with Cerner EMR preferred
Strong communication, organizational, critical thinking, and time-management skills
Ability to work independently and collaboratively within a multidisciplinary healthcare team
Key Responsibilities - Care Coordination & Case Management Coordinate patient care plans and transitions across the continuum of care (inpatient to outpatient, home health, skilled nursing)
Conduct comprehensive assessments and develop individualized care plans focused on patient goals and safe discharge
Collaborate with physicians, social workers, utilization review, rehab services, and community resources to optimize patient outcomes
Lead discharge planning, care coordination, and utilization management activities to reduce readmissions and improve outcomes
Advocate for patients and families to ensure access to services, equipment, and community support
Accurately document all case management activities and clinical notes in Cerner EMR
Monitor and evaluate patient progress, adjust care plans as needed, and escalate issues to clinical leadership
