About
Provide rehabilitative care and skilled physical therapy services to patients in their homes
Develop and implement individualized treatment plans
Collaborate with physicians, nurses, occupational therapists, speech therapists, and caregivers
Educate patients and families to promote independence and improve quality of life
Complete timely clinical documentation using electronic medical records
Qualifications
Active Physical Therapist license in New Jersey
Graduation from an accredited Physical Therapy program
Home health experience preferred but not required
Strong clinical assessment and communication skills
