About
Current state license as a physical therapist Proof of current CPR certification Current driver's license. Must have reliable transportation and insurance 1-2 years experience as a physical therapist in an acute care or rehabilitation setting Previous home care experience (preferred) Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities
Perform clinical assessments to identify patient needs and appropriateness of care Document, prepare, and review patient care plans, including progress notes and clinical record documentation Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals Instruct patients and families on transfer techniques and the use of equipment such as walkers and wheelchairs Educate patients and families regarding therapeutic procedures based on individual factors including age and condition Collaborate with an interdisciplinary team to provide comprehensive patient care
