RN Care Manager - Care Management Location: Portland, Maine Assignment Duration: This is a 13-week contract position. Schedule: 7:30 AM - 4:00 PM Job Description: The RN Care Manager is responsible for discharge planning and care coordination for patients within the care management team. This role requires effective communication, planning, and execution of patient care strategies to ensure timely and appropriate discharge processes. Responsibilities:
Conduct comprehensive patient assessments and discharge planning. Coordinate with multidisciplinary teams for patient care. Maintain effective communication with patients and families regarding discharge planning. Document care plans and progress notes in the electronic medical record (EMR). Support the care management team with on-call needs and float as necessary.
Required Experience / Certifications / Licensure:
Minimum of 5 years nursing experience. Current RN licensure required. Experience in discharge planning and care coordination. Willingness to cover care management department needs as a float.
