Registered Nurse (RN) - Homeless Shelter & Outreach Medical Case Management Position Summary We are seeking a compassionate, flexible, and self-motivated Registered Nurse (RN) to provide day-to-day medical care and case management services within a homeless shelter and community-based setting. This role includes direct client care, street outreach, and home visits for individuals in supportive housing programs.
The RN will function as part of an interdisciplinary team and will also mentor nursing students in a hands-on, community health environment. All clinical and case management activities will be performed under the direction and supervision of a Nurse Practitioner (NP). The ideal candidate is a proactive self-starter committed to serving individuals experiencing homelessness and complex health challenges with dignity, respect, and empathy. Key Responsibilities Clinical Care & Daily Shelter Support: - Provide nursing care under NP supervision - Assist in managing acute and chronic conditions - Maintain medication cart and MAR documentation - Respond to medical situations
Medical Case Management: - Conduct assessments under NP guidance - Implement care plans - Coordinate care and resources
Outreach & Community-Based Care: - Participate in outreach with outreach team - Conduct home visits - Drive van for program needs
Medication Management & Staff Training: - Train staff on medications - Ensure safe medication handling
Mentorship & Team Collaboration: - Mentor nursing students - Work within interdisciplinary team (NP, RN, LCSW, Peer Recovery Specialist)
Advocacy & Client Engagement: - Provide compassionate care to clients experiencing homelessness, mental health and substance use challenges
Additional Duties: - Other duties as assigned Required Qualifications - Active RN license - 1-2 years clinical experience - Valid driver's license and ability to drive a van - Ability to lift up to 50 lbs - Ability to work independently under supervision Core Competencies - Compassion - Flexibility - Self-starter - Strong communication skills Work Environment - Shelter, outreach, and home visits - Evenings may be required during winter season - Work with vulnerable populations
