About
Perform and document patient assessments within LPN scope of practice. Assist with patient care planning and implementation. Monitor patient condition and report changes appropriately. Support health history, admission, discharge planning, and patient education. Provide patient care and personal care support as needed. Communicate patient progress with nurses, providers, patients, and families. Document nursing care accurately and timely using EPIC. Collaborate with the multidisciplinary team to support continuity of care. Requirements Active Minnesota LPN license required. Minimum 2 years of LPN experience required. Clinic experience preferred. EPIC experience preferred. Day shift availability required. Strong communication, documentation, and patient care skills required. Ability to work effectively as part of a multidisciplinary team required.
