Full time Shift: Day-Weekend (United States of America) Address: 500 REMINGTON BLVD City: BOLINGBROOK State: Illinois Postal Code: 60440 Job Description: Participate in Interdisciplinary Rounds: Actively engage in patient care team discussions by attending rounds and providing pharmacotherapeutic recommendations to optimize outcomes. Conduct Therapeutic Drug Monitoring (TDM): Calculate pharmacokinetic parameters, interpret serum drug levels, and adjust dosing for targeted drug therapies based on clinical data. Document Clinical Interventions: Accurately record pharmacist interventions, cost-savings data, and potential outcome changes to demonstrate clinical impact. Educate Patients and Healthcare Staff: Provide medication counseling to patients, and conduct in-service education sessions or presentations for nurses, physicians, and other healthcare professionals. Prepare Drug Monographs and Policy Support: Contribute to formulary reviews, medication use evaluations, and Pharmacy & Therapeutics (P&T) committee recommendations. Support Medication Stewardship: Recommend cost-effective therapies and switch IV to oral medications when clinically appropriate, ensuring coordination with nursing. Mentor and Teach Pharmacy Students: Serve as a preceptor or co-preceptor to pharmacy students, sharing knowledge and demonstrating pharmacy best practices. Ensure Safe Medication Handling: Follow procedures for handling hazardous drugs and waste, use PPE, and respond appropriately to hazardous spills. Maintain Ethical and Professional Standards: Uphold principles of academic integrity, demonstrate leadership, and comply with institutional and professional codes of conduct. Other duties as assigned.
