This role involves admitting patients onto services and coordinating and integrating services to ensure continuity of care while facilitating the achievement of quality, clinical, and cost outcomes. The RN Case Manager negotiates, procures, and coordinates necessary services and resources for patients or families, implements the nursing process, and engages in an interdisciplinary team approach. ResponsibilitiesProvide skilled nursing and coordinate care for patients and family members in a residential setting. Observe, assess, evaluate, and plan interventions based on patients' physical and psychosocial statuses. Oversee care from Home Health Aides and CNAs, ensuring compliance with the plan of care. Ensure continuity of care through effective communication and case conferences with other disciplines and agencies. Deliver quality care, manage symptoms, and promote optimal health through patient education and referrals. Maintain accurate and complete patient records in accordance with agency standards and OASIS requirements. Participate in scheduling coordination and on-call rotations to meet patient care needs. Engage in quality improvement activities and attend in-service training as required. Schedule Typical schedule is Monday through Friday, 8 AM to 5 PM with on-call and weekend rotation. Required Experience / Certifications / LicensureCurrent RN license in the State of Montana (BSN preferred). Valid driver's license, auto insurance, and reliable transportation. 2 years of recent experience in acute care, home care, or hospice; skills in venipuncture, catheterization, wound care, and/or parenteral therapy preferred.
