LPN Home Health Opportunity We are seeking a dedicated LPN to join our Home Health team. The ideal candidate will provide compassionate care to patients in the comfort of their homes, ensuring their well-being and promoting their independence. Duties:
Administer medications and treatments as prescribed Monitor patient vital signs and health status Educate patients and families on disease management and preventive care Coordinate with healthcare providers to ensure continuity of care Document patient progress and communicate effectively with the healthcare team
