Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding. Measure and record vital signs, weight, and intake/output as directed. Assist with mobility, transfers, repositioning, and ambulation using proper body mechanics and equipment. Observe and report changes in residents' physical, mental, or emotional condition to nursing staff. Maintain accurate documentation of care provided. Ensure residents' rooms are clean, safe, and organized. Assist with meal setup and feeding assistance when necessary. Promote resident comfort, dignity, and independence. Respond promptly to call lights and resident requests.
