Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting Develop and implement individualized physical therapy treatment plans Monitor and document patient progress toward established therapy goals Collaborate with interdisciplinary care teams to ensure comprehensive patient care Educate patients and caregivers on therapeutic exercises and home programs
Required Qualifications
Active Physical Therapist (PT) license in the state of South Carolina Graduate of an accredited Physical Therapy program Current CPR/BLS certification Experience working in a Skilled Nursing Facility (SNF) setting
Preferred Experience / Skills
Prior travel or contract therapy experience Familiarity with SNF documentation and compliance requirements
