About
Follow program applicants through the intake process to enrollment. Through a combination of telehealth and home visitation, perform intake assessments of program applicants including completing the assessment form, creating a plan of treatment with the applicant, and visiting the residence to perform a home safety evaluation. Determine if the applicant meets Level of Care program requirements. Determine level of services needed to allow participants to live safely in their homes. Assess non-waiver service levels and connect clients to additional service agencies. Develop collaborative, productive relationships with healthcare facilities and partner agencies to achieve timely discharges that include optimal participant care and outcomes across the continuum. Assists with development, review and revision of departmental policies and procedures which support positive outcomes and organizational objectives. Offer input and comply with department standards of care and practice as well as quality improvement activities. Demonstrate openness to creative and innovative approaches to providing service. Comply with all DHCS reporting requirements. Completes assigned nursing training and competency validation requirements on time. Performs other duties as assigned.
Qualifications:
Graduate of accredited school of nursing. Current California Nursing RN license required. CPR certificate required. 3-5 years of Case Management experience preferred; Case Manager Certification a plus. Knowledge of Nurse Practice Act and applicable federal, state, and local regulations. Able to work independently in a dynamic, fast-paced environment. Excellent verbal and written communications including demonstrated strong interpersonal and presentation skills. Strong organizational and analytical/problem-solving skills in participant care environments. Bilingual: Spanish/English or Cantonese or Mandarin/English, preferred.
