Responsibilities
Utilize the nursing process to assess, plan, implement, and evaluate patient care
Deliver skilled nursing services in accordance with physician orders and care plans
Initiate and update care plans in collaboration with physicians and care teams
Monitor and assess patient conditions, including physiological and psychosocial changes
Respond to changes appropriately, reporting to physicians or supervisors as needed
Administer medications and treatments as prescribed, monitoring and documenting effectiveness
Collect, analyze, and interpret patient data to identify actual or potential nursing needs
Document care, observations, and patient progress accurately and timely
Collaborate with the healthcare team to ensure continuity and quality of care
Identify needs related to patient education, care coordination, goal setting, and discharge planning
Qualifications
Active Massachusetts nursing license:
Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN)
Current Basic Life Support (BLS) certification
Current TB test, PPD, or chest X-ray (as required)
Current health certificate (per contract or state regulation)
Minimum of one year of professional nursing experience preferred
