About
Review, verify, and dispense infusion and compounded IV medications for pediatric patients Ensure sterile preparation, accurate labeling, and timely dispensing of medications Conduct clinical assessments to identify potential drug-related issues and optimize therapy Collaborate with physicians, nurses, billing, and customer service on new admissions and ongoing care Assist with therapeutic drug monitoring, dosing recommendations, and lab interpretation Oversee and support pharmacy technicians and compounding workflows Maintain compliance with State Board of Pharmacy regulations, federal guidelines, and PHS policies Document all pharmacy activities clearly and accurately in patient records Support pharmacy SOPs and quality initiatives in partnership with Pharmacy leadership Provide training and mentorship to technicians and pharmacists as needed Maintain working knowledge of infusion pumps and home infusion devices

Bachelor of Science in Pharmacy (or PharmD) Active pharmacist license in good standing in the state of practice Strong attention to detail and commitment to patient safety Ability to thrive in a fast-paced, regulated environment
Pediatric Home Service is a pediatric home health agency that has provided high quality DME and nursing services to children that are medically fragile since 1990. Our services allow our clients to remain and thrive in their homes, schools, and communities. The majority of the clients we serve are age's birth through 21 years old and have a variety of technologies that might include a trach, ventilator or g-tube. Benefits:

The starting rate for this role is $125,000 annually, based on experience and qualifications.
