About

Physical Therapy - PT SNF Permanent Physical Therapist (PT) opportunity in a Skilled Nursing Facility (SNF) setting working Monday through Friday with 40 hours per week. This role supports rehabilitation services within a 115-bed skilled nursing facility located near Villanova University and features a dedicated physical therapy gym. The Physical Therapist will evaluate and treat residents with mobility, orthopedic, neurological, and functional impairments while collaborating with interdisciplinary teams to improve patient outcomes and quality of life. Requirements:
Active Physical Therapist license required Minimum 2 years of Physical Therapy experience required Skilled Nursing Facility (SNF) or Long-Term Care experience preferred Strong knowledge of rehabilitation treatment planning and functional mobility training Experience with geriatric and post-acute patient populations preferred Ability to work independently and collaboratively within an interdisciplinary team Strong documentation and communication skills required Must be willing to work one weekend day per month BLS certification required
The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide rehabilitative therapy services focused on improving strength, mobility, balance, endurance, and functional independence. Responsibilities include performing therapeutic exercises, gait training, transfer training, mobility assessments, and patient/family education while documenting patient progress accurately in accordance with facility and payer requirements. The PT will collaborate with nursing staff, physicians, therapy teams, and interdisciplinary providers to support patient-centered rehabilitation goals and ensure high-quality care delivery within the skilled nursing environment.
