Summary We are seeking an experienced, clinically strong nurse to support our interdisciplinary care team serving senior care patients in skilled nursing facilities. This role is hands-on and patient-facing, providing direct clinical care, supporting the APRN, and serving as a clinical anchor for the team. The ideal candidate brings strong SNF experience, sound clinical judgment, and the ability to collaborate closely with providers, facility staff, patients, and families. Essential Duties & Responsibilities Key Responsibilities Clinical Care
Round daily on Align senior care patients Assist APRN with clinical assessments and interventions Perform Foley and suprapubic catheter changes Complete wound and dressing changes Start IVs and administer IV medications, including antibiotics Administer IM medications (e.g., Lasix, antibiotics, prednisone, dexamethasone) Provide nebulizer treatments Complete timely and accurate nursing documentation
Care Coordination & Team Collaboration
Assist Care Ally with Health Risk Assessments Support facilities with coordination of care and follow-up Participate in clinical rounds and provide patient reports as needed Engage in family conferences when requested Communicate patient updates with families and facility staff
Qualifications Required
Active RN or LPN license in good standing Strong IV therapy and medication administration skills Excellent documentation and communication skills Ability to work independently and make sound clinical decisions Willingness to work 10:00 AM – 7:00 PM and flexible scheduling, including occasional weekends Preferred
Previous leadership experience (Unit Manager, Shift Supervisor, ADON, or similar) Skilled Nursing Facility (SNF) experience Experience serving as a clinical lead or resource for staff Multilingual abilities based on facility needs Flexibility to rotate schedules (e.g., Mon–Fri one week, Tues–Sat the next)
