Who You Are The Intake Assessment Coordinator is responsible for managing and coordinating screening assessments of individuals seeking inpatient treatment at Gentle Heart Recovery-managed facilities.
What You'll DoIn collaboration with marketers and outreach, answer intake calls and voicemail messages and conduct pre-screening assessments of all potential patients to develop criteria for the most appropriate level of care (detox or rehabilitation). Report information on expected patients for admissions, with an emphasis on Airtable updates and referral paperwork updates.
Educate referral partners and other community resources seeking information on our services, facilities, availability, admissions, and treatments.
Assist marketers by following up with prospective patients that have been approved from treatment and need to secure transportation, with an emphasis on inputting information related to transportation services timely into Microsoft Teams and Airtable,
Coordinate with potential referrals to develop the criteria and documentation for which level of care is most appropriate (detox or rehabilitation placement). Verify insurances of prospectives patients, utilizing the ePACES NY Portal for NY Medicaid insured individuals. In the event of third-party or commercial insurance, submit a request for the Verification of Benefits (VOB) process with the desired facility for admissions approval. Inform the patient that all insurance deductibles and co-payments are the responsibility of the patient.
When needed, initiate case conferences with the Director of Admissions or Director of Clinical Reimbursement for admissions approval. Follow up to ensure the completion of referral paperwork sent from the marketers and/or other referral sources on the Airtable paperwork grid Frequently communicates with and informs the Admissions Department on the status of all expected patients throughout the day. Posts patient screening assessments to applicable facility. Report all significant patient medical or clinical concerns to the Intake Manager, the Medical Director, and/or the Executive Director for review and guidance before allowing patients to arrive at facilities for admission. Coordinate all patient transportation requests with the Transportation Coordinator and post updates to the patient transportation chat in Microsoft Teams. Review the intake grid on Airtable for the daily expected number of patients and patient reservations to maintain continual awareness of the status of incoming patients. Inform all patients about the facility policies and procedures including, but not limited to the facility setting, admissions process, and treatment programs. Adhere to transportation safety guidelines under all governing agencies as it relates to job functionality. All other duties as assigned.
What You BringMINIMUM QUALIFICATIONS:
High School Diploma or Three (3) years of experience as an Intake Coordinator (or equivalent position) working in the substance use treatment
SKILLS:
Knowledge and competence of local communities and
Outstanding interpersonal skills, with an emphasis on the capability to successfully build professional relationships. Excellent written and verbal communications
Strong time management capability. Ability to multitask and prioritize effectively.
Ability to utilize the Microsoft Office Suite and other necessary computer and technology platforms. Adept at strategizing and executing business
