Conduct evaluations for patients in both inpatient and outpatient environments and craft tailored treatment plans Implement physical therapy interventions that prioritize mobility, strength, balance, and regaining daily function Collaborate closely with a team of nurses, physicians, case managers, and fellow rehab specialists to enhance patient outcomes Guide patients and families through therapeutic milestones, discharge planning, and home exercise regimens Keep detailed records of evaluations, treatments, and patient progress in line with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure seamless care delivery Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program Certifications
State licensure as a Physical Therapist CPR certification is preferred Any specialized certifications (e.g., geriatric, neurological) are highly regarded, though not mandatory Skills
Relevant experience is strongly preferred Capability to manage patients with diverse medical and mobility challenges Excellent communication and collaborative skills Familiarity with digital documentation and clinical record-keeping systems Benefits
