What You'll Do
Provide routine nursing care and health monitoring Administer medications and treatments as directed Observe and report changes in health or behavior Assist with daily living activities (ADLs) like hygiene, mobility, and nutrition Support implementation of individualized care and behavior plans Maintain accurate documentation and medication records Ensure proper medication storage, handling, and inventory Respond to emergencies and provide immediate care when needed Collaborate with RNs, providers, and interdisciplinary teams
What You'll Bring
Active LPN/LVN license (multi-state preferred if applicable) Graduate of an accredited LPN/LVN program 1+ year of nursing experience preferred Experience with IDD, developmental disabilities, or long-term care preferred Strong attention to detail and observation skills Ability to work independently and communicate effectively Familiarity with EHR systems and basic computer tools Valid driver's license and reliable transportation BLS certification
