About
Review, interpret, and process physician medication orders accurately and efficiently Prepare, dispense, and monitor medication therapies in accordance with applicable laws and best practices Monitor drug therapy, including lab values and levels; recommend or implement appropriate adjustments Conduct medication reconciliation and ensure accurate documentation in the Medication Administration Record (MAR) Ensure all medication charges and reports are completed accurately and daily Maintain pharmacy operations, including Pyxis system management and drug database accuracy Collaboration & Communication
Collaborate with physicians and nursing staff to optimize medication therapy and formulary use Communicate drug-related information clearly to patients, families, and interdisciplinary team members Partner with providers to identify cost-effective medication alternatives when appropriate Actively participate in team conferences and interdisciplinary care planning Patient & Staff Education
Provide medication education to patients and families and document all interactions Deliver in-services and ongoing education to nursing and clinical staff Support orientation, training, and competency validation of pharmacy and nursing personnel Regulatory Compliance & Quality Improvement
Ensure compliance with DNV, CMS, state, and federal pharmacy regulations Maintain a clean, organized, and inspection-ready pharmacy environment Participate in Performance Improvement (PI) initiatives to enhance service quality and efficiency Attend and contribute to monthly department and staff meetings Process & Systems Management
Follow established protocols, including:
Automatic therapeutic substitutions IV to PO/PT conversions
Manage Pyxis system functions, including:
Adding medications Ensuring accurate medication descriptions Adding and maintaining nursing user access
Reconcile reports including discrepancies and overrides Professional Responsibilities
Provide on-call pharmacy coverage as needed Communicate effectively with vendors and external partners Address and resolve concerns from patients, families, and staff professionally Support departmental goals and perform additional duties as assigned POSITION QUALIFICATIONSEducation
Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited College of Pharmacy Advanced degree or completion of an accredited hospital pharmacy residency preferred Experience
Experience in a hospital or pediatric setting preferred Strong knowledge of DNV, CMS, and state regulatory requirements Licensure & Certification
Current, active license as a Registered Pharmacist in the state of Texas IV Certification Preferred
