Following the Plan of Care – Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences. Assisting with mobility and safety – Helping patients transfer, move, and ambulate safely—while maintaining a calm, clean, and secure environment. Observing and reporting – Noticing and promptly reporting any changes in the patient's condition or behavior to the RN, supporting proactive care. Practicing infection control – Following all safety and hygiene protocols to protect the health of patients, families, and teammates. Respecting privacy and dignity – Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
Skills and Qualifications
Completion of a state-approved Certified Nursing Assistant (CNA) training program. Active CNA certification in the State of Texas. High school diploma or equivalent required. At least one year of experience in hospice, home health, or a similar care setting is preferred. Compassionate presence and strong communication skills. Basic understanding of hospice care principles and a heart to serve. Ability to read, write, and communicate clearly in English. Comfortable using a smartphone for communication and documentation. Valid Texas driver's license, reliable transportation, and current auto liability insurance
Where You'll Work In this role, you'll serve patients wherever they call home—whether in a private residence, assisted living, nursing facility, or memory care setting. You'll provide personal care in varied environments, both indoors and outdoors, and may be exposed to infectious diseases and bodily fluids as part of your essential work. You'll also regularly connect with your local care team to stay aligned on patient needs and ensure care is consistent, respectful, and personalized.
