Job Description Southern Rehab is seeking a Physical Therapist to join our team . This full-time role involves providing exceptional care in an outpatient setting using the latest evidence-based techniques. We believe in collaborative work with other healthcare professionals and colleagues to ensure dynamic treatment for our clientele.
Assess patient goals and limitations to develop a therapeutic plan of care and therapy timeline
Carry out therapeutic interventions using the latest evidence-based techniques that appropriately address a patient’s specific needs and concerns
Document initial evaluations, daily notes, progress notes, and discharge summaries using Raintree electronic medical records
Maintain patient records in accordance with federal and state regulations and company policy to ensure excellent standard of care
Provide regular updates to patients, physician’s and other therapists involved in the patient’s care
Communicate effectively with team of healthcare providers and colleagues to ensure an exceptional continuum of care
Educate patients, families and caregivers on a home exercise program that complements therapy sessions
Participate in continuing education and other professional development opportunities to remain knowledgeable about new and emerging therapies.
Promote health and wellness with community outreach during outdoor recreational events such as high school sports, biking, and running races
Qualifications
Graduate with a Masters or Doctorate from an accredited Physical Therapy program
Current state licensure
CPR Certification (or will complete upon hire)
New or recent graduates are welcomed and encouraged to apply
1+ years of outpatient orthopedic experience within a private clinic or hospital preferred (new graduates welcome to apply)
Strong understanding of biomechanics and functional movements highly valued
